Submit Documents
Bring all required documents to the Registrar's Office during office hours for evaluation.
Assessment / Interview
Incoming Grade 7 and Grade 11 students may be required to undergo an assessment or interview.
Enrollment Confirmation
Once accepted, proceed to the cashier (if applicable) and complete your enrollment form.

Grades 1 – 6
- PSA Birth Certificate (original + photocopy)
- Report Card / SF9 from previous school
- Certificate of Good Moral Character
- 2x2 ID photos (2 pieces)
- Barangay Clearance
Junior High
Grades 7 – 10
- PSA Birth Certificate (original + photocopy)
- Grade 6 Report Card / SF9
- Certificate of Good Moral Character
- 2x2 ID photos (2 pieces)
- Form 137 / Transcript of Records
Senior High
Grades 11 – 12
- PSA Birth Certificate (original + photocopy)
- Grade 10 Report Card / SF9
- Certificate of Good Moral Character

- SHS Voucher (if applicable)
